A lethal dose of the medical sedative midazolam, mixed with a cup of early morning coffee, caused the death of Saugerties physical therapist Thomas Kolman in 2011, forensic pathologist Michael Sikirica testified Thursday. But under questioning by defense attorney Gerald Shargel, Sikirica told an Ulster County jury that the final autopsy took more than three and a half years to finish, that he didn't first rule midazolam as the cause of Kolman's death and that some errors occurred along the way.
